How to Access Services
Getting started with our
Community Health Worker program is simple
Must have MEDICAID Insurance
Households in our service area
Individual/Family seeking health and wellness support
Parents with young children or infants
Families managing chronic health conditions
Households needing care coordination
Anyone referred by partner organizations
What to Expect:
After enrollment, a dedicated Community Health Worker will be assigned to your family. They will schedule an consultation to assess your family's needs and develop a personalized care plan. Regular visits and check-ins will follow to provide ongoing support and resources.
All services are provided at no cost to families that have MEDICAID. (If you have other insurance, then please sign-up to be placed on our waitlist). Our CHWs speak multiple languages and respect the cultural backgrounds of all families we serve. (upon request)
